*It is hereby understood that a $100 deposit per child is required for enrollment. By submitting this form, you are agreeing to pay the balance on or before May 15, 2013. Interest will be charged at the rate of 1.5% per month on the balance due after May 15, 2013. Kids Playing Summer Camp reserves the right to cancel enrollment if all fees are not paid in full by that date.
Cancellation Policy: Deposits are nonrefundable. Camp fees, less the deposit are refundable until May 15, 2013. NO REFUNDS WILL BE ISSUED FOR CANCELLATIONS AFTER MAY 15. Prorated refunds will be considered for medical reasons only.
